Centenarians Don’t Escape Disease, They Compress It

Perls’ research group has found that instead of the common aging narrative in which the older a person gets, the sicker they become, centenarians tend to show the opposite pattern, staying markedly healthy until very late in life.
Massachusetts centenarians studied by Perls’ team lived roughly 96 percent of their lives functionally independent and in good health, with morbidity compressed into a small window near the end. Among the smaller group who reach 110 and beyond, the effect is even more pronounced.
Supercentenarians, on average, do not experience age-related disease until the final five percent of their exceptionally long lives. Geriatricians refer to this as the compression of morbidity, a concept first proposed by physician James Fries in 1980 in the New England Journal of Medicine and later confirmed empirically in centenarian cohorts. The Georgia Centenarian Study found that centenarians develop compensatory behavioral and coping skills and personality traits that support life satisfaction and mental health well into extreme old age, with positive attitudes and survival traits showing up even among those who are frail. As one of the study’s authors put it, healthy aging is in the eye of the beholder.

A 2025 latent profile analysis of participants in the Iowa Centenarian Study identified two distinct psychosocial profiles among its 138 subjects. The smaller group, representing about 19 percent of the sample, was characterized by long-term marriages, higher education, lower neuroticism, and secure attachment, and differed from the larger group on marriage length, education, and attachment security with strong statistical significance.
Attachment security in particular showed one of the largest effects in the entire study, a detail that rarely makes it into popular longevity coverage but points to something more durable than simply having friends: a stable, trusted relational base sustained over decades.
Isolation works in the opposite direction with equal force. Research using the Chinese Longitudinal Healthy Longevity Survey noted that centenarians represent a unique model for studying healthy aging precisely because social isolation and loneliness, while conceptually distinct, both carry measurable health consequences among the oldest-old population.
The practical implication is that relationship quality and relationship duration function less like emotional nice-to-haves and more like structural supports, comparable to diet or exercise in their downstream effects on survival.
Autonomy and a Reason to Get Up
A less discussed but recurring theme in the qualitative literature on centenarians is the persistence of a sense of purpose and self-directed identity late into life. Interview-based studies describe centenarians as unapologetically themselves, often stubborn about maintaining independence in daily routines even as physical capacity declines. That stubbornness is not incidental.
Continued engagement in decisions about one’s own life, from managing a household task to maintaining a daily walking routine, keeps cognitive and physical systems active in a way that passive care cannot replicate. Geriatricians increasingly describe this as one of the more overlooked levers in healthy aging: not what a person is prevented from doing, but what they insist on continuing to do.
The Blue Zones Reckoning
No discussion of centenarian research in 2026 is complete without addressing the credibility crisis that has upended the field’s most famous public-facing concept. The term Blue Zones, popularized by writer Dan Buettner after National Geographic reporting on Sardinia, Okinawa, and later Costa Rica, Greece, and Loma Linda, California, became shorthand for regions with supposedly extraordinary concentrations of centenarians tied to diet and lifestyle.
Demographer Saul Justin Newman, now at University College London, spent years auditing the underlying age-verification data behind those claims and found the numbers did not hold up. Newman’s analysis found that a strong correlation exists between high poverty rates and inflated reports of extreme longevity, and that Eurostat records dating back to 1990 placed Ikaria, one of the most celebrated Blue Zones, 109th out of 128 European regions for old-age life expectancy.
Newman told The Conversation that, by his estimates, at least 72 percent of Greek centenarians were dead, missing, or involved in pension-fraud cases, and a Japanese government review found that 82 percent of the country’s registered centenarians, roughly 230,000 people, were missing or dead. A 2008 study in Costa Rica found that 42 percent of centenarians had misrepresented their age in an earlier census.
The mechanism is bureaucratic rather than mystical. Only 18 percent of supercentenarians who have been exhaustively validated worldwide actually have a birth certificate on file, which means most extreme-age claims rest on secondhand documentation, oral history, or record systems with strong financial incentives for families to keep a deceased relative’s pension active.
As Newman summarized it, the secret to living to 110 was: don’t register your death. The Annals of Improbable Research awarded Newman the first-ever Ig Nobel Prize in Demography in September 2024, specifically for demonstrating that supercentenarian age records show patterns consistent with clerical error and pension fraud rather than exceptional biology.
This does not mean that nothing useful came out of Blue Zones research. Diets rich in vegetables, regular movement built into daily life, and strong community ties remain well supported by evidence independent of the specific regional claims. It does mean that treating five geographic locations as uniquely magical longevity hotspots was, at minimum, built on shakier demographic ground than a decade of bestselling books suggested.

Why the Anti-Aging Industry Keeps Missing the Point
Perls has been pushing back against commercialized longevity claims for longer than most of the current supplement market has existed. In 2004, he published a peer-reviewed paper in the Journals of Gerontology titled Anti-Aging Quackery: Human Growth Hormone and Tricks of the Trade, warning that the marketed shortcuts to extreme longevity were, if anything, more dangerous than ever.
Two decades later, the specific products have changed, from HGH to NMN to peptide stacks, but the underlying pattern he identified has not: an industry selling biological interventions faster and cheaper than the science supporting them can be verified.
The centenarian research summarized above points to a genuinely inconvenient conclusion for that industry. The traits most consistently associated with reaching 100, namely genetic background, personality structure, relationship depth, and sustained autonomy, are either inherited or built slowly over a lifetime of behavior.
None of them can be accelerated with a purchase. That is not a marketable message, which may explain why it gets far less attention than the next headline ingredient promising to add a decade to human lifespan.
